PROBATION  CONTRACT

I.    GRADING PERIOD

       The terms of this contract will apply to the       Six-Week Grading Period of School Year 2002-03.

II.   PARTIES

      This document represents an agreement between the following parties:

IV.  AGREEMENTS

As student, I choose to go on academic probation and understand this will be only grading period this year that this option is available to me. I understand that I will not be allowed to participate in CIF contests until I have remedied the deficiency that has led to my ineligibility. I agree to the terms of this contract as stated above and understand that breaking the terms of this contract may result in my not being able to play. I also acknowledge my responsibility to get a completed Progress Report to the Athletic Director by Friday of each week of my probation period.  I understand that failure to do this will result in my not being able to play.

As teacher, I agree to permit this student to go on athletic probation. I also agree to notify the Athletic Director if the student has not upheld the terms of the Contract.  I further agree to update this student’s grade weekly and to report this updated progress (or lack of) on a Progress Report supplied by the student. 

As parent, agree to allow my student to enter athletic probation and to the terms of the contract.
